What this assumes

  • Uses the 2026 Part B premiums and IRMAA brackets published by CMS, based on your 2024 modified adjusted gross income.
  • Premium amounts are the exact figures CMS publishes for each bracket.
  • Married filing separately has its own, stricter brackets. This calculator covers single and joint filers.
  • The Part D surcharge is added on top of whatever your Part D or Medicare Advantage drug plan charges.
  • Income dropped because you retired or another life change happened? You can ask Social Security to use the newer figure (form SSA-44).
  • CMS announces the 2027 amounts around November 2026.
  • An estimate from published figures and the numbers you enter, not a quote or an eligibility decision.

Common questions

What income triggers IRMAA in 2026?

Modified adjusted gross income above $109,000 for a single filer or $218,000 for a joint return, taken from your 2024 tax return.

Why does 2026 IRMAA use my 2024 income?

Social Security uses the most recent tax return the IRS has, which is usually from two years earlier.

Can I appeal IRMAA if my income went down?

Yes, after certain life-changing events such as retirement, a reduction in work hours, or the death of a spouse. You file form SSA-44 with Social Security and ask them to use the newer income.

Does IRMAA apply to Part D and Medicare Advantage drug coverage too?

Yes. An extra Part D amount is added at the same income levels, on top of what your drug plan or Medicare Advantage plan charges.

How much is the Part B premium with IRMAA in 2026?

It runs from $284.10 a month in the first IRMAA bracket to $689.90 at the top, against the standard $202.90. The Part D surcharge adds $14.50 to $91 a month on top of your drug plan’s premium. If you and your spouse both have Medicare, each of you pays it.

Can a Roth conversion or selling a home raise my IRMAA?

It can. One-time income, such as a Roth conversion or a large taxable gain on a sale, counts in your modified adjusted gross income for that year, and two years later it can push your Medicare premiums into a higher bracket. Talk with a tax professional before a large transaction, and run the figures here first.

Do I pay IRMAA every year?

Only in years when your income is above the threshold. Social Security looks at your income again every year using your newest tax return, so if your income falls below the brackets, the surcharge stops. If it stays high, the surcharge continues, and the brackets themselves are updated each year.

Does IRMAA apply if we are married filing separately?

Yes, and the brackets are stricter. Married filing separately has its own IRMAA table, which this calculator does not cover; it handles single and joint filers. If you file separately, check the CMS 2026 Part B premium fact sheet, or call us and we will look up your amount with you.

When will the 2027 IRMAA brackets be announced?

CMS usually announces the 2027 Part B premium and IRMAA amounts around November 2026, based on 2025 tax returns. Until they are published, this calculator uses the 2026 figures, and we update it once CMS releases the new table. Your Social Security letter will also confirm your own amount.
